  • Required Documents: Due with this form by April 1st 2026:

    1. Valid proof of address: Recent bank statement, utility bill, or lease agreement. 

    2. Proof of age and guardianship must be submitted along with this form.  Students must turn five years old by December 1, 2026.

    3. New residents must submit their request(s) within 30 days of moving into the district and provide proof of residency and a copy of the child’s original birth certificate.
